Not having a written set schedule including hours of operation during the day

Leaving details or fixtures/materials undecided when you start building

Not Setting clear expectations with the workers about how they should operate and respect the home

A clear payment schedule associated with specific milestones

A clearly defined written scope of work detailing what is included and what is NOT included

Not obtaining copies of company’s Liability and workers comp insurance 

Having the homeowner purchase materials or non-reputable sources

Not having a mutually agreed up final punch list that defines the end of the job

Not checking references

Avoiding a permit

Not having a clear plan if there are pets in the house

Not discussing the chain of command or method of communication

All change orders in writing and signed BEFORE the work is done

And just to make sure you get this one -  ALL CHANGE ORDERS IN WRITING AND SIGNED BEFORE THE WORK IS DONE.
